Blood thinners like heparin are given in few interval of time for the first few days to stop further formation of the clot.
If the stroke is large, then dead tissue will swells up and can pushes the medulla downwards, blocking its vital functions and causing death. In this condition, a neurosurgeon removes a part of the skull in order to allow part of the brain to swell up.
